ActivityPro Family Portal Terms and Privacy Policy

What is the Family Portal?
The ActivityPro/Providence Healthcare Family Portal is a private site for you and your family to communicate with each other and the family member staying in our home. The Family Portal provides you with a unique ability to view your loved one’s activity schedule, share comments and pictures, as well as see the quality, quantity and variety of activity programming offered from month to month. The Portal also provides Providence Healthcare with the ability to inform family members of upcoming important events.


Who has access to administer and control the Family Portal?
If the resident, or the power of attorney gives consent to create a Family Portal page, one will be created along with a username and password for this person. This person will be called the “family portal admin”. The Family Portal admin is responsible for adding and removing users (other family members) from accessing the Family Portal.


What are the responsibilities of family users of the Family Portal?
The power of attorney and family members are all responsible for the content within the Family Portal. Providence Healthcare will not actively police or monitor the posts, pictures or comments made by the users of the Family Portal. If there is questionable material that is offensive to members of the family, it is up to the power of attorney and family members to remove that material. Removal of content is easy and any user can do so. The expectation is that any and all disputes about the Family Portal or the content within it must be resolved by the power of attorney and family members. Treat this space the same as you would a bulletin board within your home.

What are the responsibilities of the home/organization with the Family Portal?
The HOME does have access and can view information posted within the Family Portal, but are not responsible for curating, policing, or removing users. For example, if a family member is removed by the power of attorney without notice, HOME is not responsible for resolving the potential conflict. This is a family matter and should be treated as such.
Should family members be unable to resolve issues that may be disruptive to the resident’s well-being, Providence Healthcare reserves the right to limit or discontinue access to the family portal until such time as resolution is established. Providence Healthcare also reserves the right to limit or discontinue access to the Family Portal should this communication channel be in contravention of the Values of the Home, the Code of Conduct, or the Resident Bill of Rights.

Why do I not see my Mom/Dad participating in Activities in the Family Portal?
The Family Portal automatically displays reports on the number and types of organized activities your loved-one is participating in. Sometimes you may notice that no activities were recorded for your MOM/DAD. There are many reasons why this may be the case:
- They chose not to attend
- They may be independently active with their own activities (making it difficult for our staff to record)
- There may be a small delay between when the activity occurred and when our staff recorded it in our system.

It is important to remember if there are no, or few activities displayed in the Family Portal, there may be many other reasons. Use the Family Portal as a gateway to ask our staff questions about our programming. We want you and your family to be well informed about day to day life at Providence Healthcare. Over time, we think you’ll be amazed at the quality of care we give and the amount of work we do to enrich your loved one’s lives.

What is ActivityPro?
ActivityPro is a 3rd party software provider that helps long term care facilities track and organize Activity and Recreation Therepy programs. ActivityPro licenses software to long term care facilities who can then choose to allow family members to access the Family Portal.

What information does the ActivityPro Family Portal collect and what do you use it for?
ActivityPro collects and stores your name, username, password, email, IP address, uploaded photos and messages, and activity-related data associated with your loved one.

This information is used to personalize your Family Portal and to allow secure logins to the Family Portal. Your personal information is never shared outside ActivityPro, or Providence Healthcare.

Providence Healthcare or ActivityPro may occasionally contact you via email to share newsletters, updates, and technical maintenance announcements.

How does ActivityPro protect your information?
We implement a variety of security measures to maintain the safety of your personal information when you access your personal information.

We use a secure server. All supplied information is transmitted via Secure Socket Layer (SSL) technology and then encrypted into our Database to be only accessed by those authorized with special access rights to our systems, and are required to keep the information confidential.

Do you use cookies?
Yes. Cookies allow ActivityPro to recognize your browser and capture and remember who you are for the next time you visit. This allows us to save your preferences for future visits and compile aggregate data about site traffic and interaction so we can offer a better experience in the future. We may contract with a third-party service to assist us in better understanding our visitors. These service providers are not permitted to use the information collected on our behalf except to help us conduct and improve our business.

Occasionally, at our discretion, we may include or offer third party products or services on our website. These third party sites have separate and independent privacy policies. We therefore have no responsibility or liability for the content and activities of these linked sites. Nonetheless, we seek to protect the integrity of our site and welcome any feedback about these sites.

California Online Privacy Protection Act Compliance
Because we value your privacy we have taken the necessary precautions to be in compliance with the California Online Privacy Protection Act. We therefore will not distribute your personal information to outside parties without your consent.
As part of the California Online Privacy Protection Act, all users of our site may make any changes to their information at anytime by logging into their control panel and going to the ‘Edit Profile’ page.

Childrens Online Privacy Protection Act Compliance
We are in compliance with the requirements of COPPA (Childrens Online Privacy Protection Act), we do not collect any information from anyone under 13 years of age. Our website, products and services are all directed to people who are at least 13 years old or older.

Agreement:
Before access is given to the Family Portal, we want to ensure you fully understand and agree to the guidelines listed in this document. If a situation was not explicitly covered, it should be assumed that it is a family matter for the family to resolve. The Family Portal is your space and you are responsible for it. Providence Healthcare gives you the space to use, but also reserves the right to revoke this space at any time.
